I think that nicknames should have appropriate meanings. My sister Allie is 'twerp' because she is a twerp. Anna is 'Stinkerbell' because she's cute and stinky in a sisterly sort of way, and because of the rhyme I made up in her honor: 'Stinkerbell, you smell like you fell down a well.'
Computer user names often become nicknames because the person using them becomes associate with those names. My friend Matt is Thirteen on a bbs, so I refer to him as Matteen as a combination of such. Dana is Skamp or Coygirl. Dan is Swankboy. Jill is Jillybean. Randy is Randybus (Randy + Cerebus). I've been known as Rowan (as much for my sometimes red hair as the fact that I'm a big Anne Rice fan) and Pixie because I have fey characteristics.
Pretty much everyone has a nickname in my universe, whether they know it or not. I assign names to people like Mr. Smoove or Mr. Wrong based on my experiences with them. Overlord Carol, while gender-confusing, is terribly appropriate if you've ever had the experience of working for her.
So it is terribly perplexing when I meet people that have nicknames that have no apparent explanation. The appropriate response to 'Why do they call you T-Rex?' is not "Uh, I dunno. They just do."
The answer I would be satisfied with is "Because I have short stubby arms and big teeth, and my roommates leave goats chained to a pole in the living room to distract me when I'm stomping around and feeling moody."
